A serous fluid collection, also known as a seroma, can develop as another potential risk following surgery. This is the area beneath the skin where the facelift was done, where a collection of bodily fluid, not blood, accumulates. Since water makes up 65% of our bodies, a tiny amount of watery fluid may accumulate in the area between the muscles and the skin flap until the skin adheres to the deeper layer. Since seromas are extremely rare, tiny drainage tubes that enable passive suction are inserted to stop fluid buildup. Spaghetti noodle-sized drains emerge behind the hairline of the neck and are removed a few days later after the skin has reattached to the muscles. If a patient does experience a squishy area of fluid, it is usually treated easily by aspirating the fluid directly with a needle. Early detection and treatment have no effect on recovery or outcomes.

There is a chance of infection with every surgical procedure. All surfaces and environments are home to bacteria and viruses, which are living microorganisms that can cause an active infection once they reach a certain stage. Because the face has a large, healthy blood supply and the surgical environment is sterile, infections following facelift surgery are extremely uncommon. I don’t use any facial compression dressings following facelift surgery because I think that decreased blood flow and increased inflammation are significant risk factors for poor skin healing. Although facial compression dressings have historically been used to prevent hematomas following facelift surgery. I think they are unnecessary with current techniques and actually do more harm than good.

I have never had a hematoma in a patient who did not wear a compression band as of this writing. Additionally, I firmly support the regular application of hyperbaric oxygen therapy (HBOT) following facelift surgery. For many years, HBOT has been used to help heal wounds. The treatment makes it possible for the blood to carry much more oxygen. It reduces the possibility of poor skin healing and permits quicker wound healing. The first course of treatment is conservative if a patient has poor wound healing. Both HBOT and medical-grade skincare products, such as silicone scar cream, can be used. If necessary, laser energy devices or even surgical scar revision can be used later.

Furthermore, permanent facial weakness brought on by damage to the facial nerve is one of the most dreaded side effects of facelift surgery. If the facial nerve, which regulates and moves the facial muscles, is damaged, it may permanently weaken the face. Fortunately, reports indicate that this complication occurs between 0.9% and 0.2% of the time, making it incredibly rare. Permanent facial weakness has never been a problem for any of my patients. It’s interesting to note that the type of facelift has no bearing on the risk of facial nerve damage. There is no evidence that a minimally invasive procedure like a SMAS mini-facelift, which is far less complicated, carries a higher risk of nerve damage than an extensive facelift like an extended deep plane facelift. Regardless of the method employed, the surgeon’s skill and experience are the primary factors influencing the risk of nerve damage. A skilled deep-plane facelift surgeon is far less likely to harm a facial nerve than an inexperienced surgeon using a very basic facelift technique.

Lastly, a risk associated with any cosmetic procedure is that, even if the procedure is done correctly and the patient looks better, there may be an unpredictable healing area that does not turn out as planned. For instance, a patient may observe that one skin fold or wrinkle is still visible to them. Alternatively, a different patient may observe that one side of the neck appears somewhat fuller than the other. These results are usually the consequence of unpredictable healing following surgery. In 1-2% of cases, a patient may benefit from a small, focused surgical procedure to enhance a facelift area. A non-surgical skin tightening procedure like radiofrequency microneedling may be the best choice if the desired change is particularly small and not worth having a surgical procedure done for such a minor improvement.
