It is considerably harder to get the nostrils to look the same with revision rhinoplasty. This could be due to scar tissue, the loss of supporting cartilage during the first rhinoplasty, or unsuccessful attempts to make the nostrils smaller.
The tip cartilages, the skin that covers them, the position of the nasal septum, the height of the bones of the maxilla (upper jaw), and the muscles and fat around the cheeks all work together to shape the nostrils. The nostrils do have muscles that can make them flare (the anterior dilator naris and the alar nasalis), but these muscles are quite thin and don’t usually change the shape of the nostrils very much. When rhinoplasty removes, repositions, and inserts cartilage to the tip area, the form of the nostrils might change right away or over time as they heal. During rhinoplasty, swelling can make it hard to see the changes and hard to guess what will happen following the surgery.

For mild to severe nose retraction, cartilage grafts that can push the nasal border down are used. When needed, this might be done along with moving the alar cartilage over the nostril. If the retraction is really bad, the inside lining of the border of the nostril also needs to be lengthened. In these situations, composite grafts made from skin and cartilage from the ear are employed. The skin is put inside the edge of the nostril to make it easier to pull down, and the cartilage gives it more stability.
If the columella is too drooping or off-center, the bottom margin of the septum needs to be straightened and trimmed. Then, after straightening, the medial crura, which are parts of the tip cartilages, are lifted to meet the now shorter septum and sewed there. To make sure the repair lasts, you may also need to take off some of the skin on the inside. This is a different way to do the “tongue-in-groove” method.
