The main age group would be younger, possibly as young as five, but generally in the range of early teens. And then sometimes, rarely, you’ll see people in their thirties or forties.

What Kind Of Surgery Is Needed For Correction?

There are two main procedures for protruding ears.

The Davis Procedure

There is something called the Davis procedure, and I’ll go through that with you.

The Davis procedure corrects excess cartilage in the ear, which causes the ear to stick out from the head. The conchal cartilage, inside the ear, looks like a kidney bean-shaped bowl.

The Davis procedure removes extra tissue from the base of this bowl that allows the air to be repositioned so it lies in a more normal position closer to the head.

And that was the Davis way. Thatโ€™s what we call hypertrophy of the conchal bowl cartilage.

The Mustarde Procedure

The second problem is referred to as a lack of an antihelical fold. We should all have a little crease or a fold right here, which determines where the tip of the ear sits.

If you don’t have an antihelical fold there, the ear will pop out and stick out. Weโ€™re discussing the top of the ear, not the entire ear that we addressed in the Davis procedure.

The Mustard Procedure recreates the loss of the antihelical fold.

The Mustarde technique folds the outer ear where there is not a fold. This fold is created by measuring, marking and sewing in place with permanent sutures to create a more natural-looking fold.

And itโ€™s pretty much done with permanent stitches that fold the top of the ear back in and create that antihelical fold.

How Long Does Otoplasty Surgery Take?

Again, the length of an otoplasty depends on what you need to do. If you’re doing the Davis and the Mustarde procedure, you’re probably going to be two-and-a-half to three hours. The other two probably take half that time. So, it would take an hour and a half each if done separately.

How Painful Is Otoplasty?

The pain from otoplasty is more achy than sharp or deep-cutting. Most people have this sort of discomfort for a few weeks or so.

After the surgery itโ€™s pretty much a turban that wraps around and pushes on the ear. And that stays on for a day or so. Once thatโ€™s taken off, people can use ice, and it is a wonderful thing in terms of relieving pain. 

Not only does it numb the area, but it also reduces swelling, and more swelling means more pain. 

What Is The Typical Aftercare After Otoplasty?

An otoplasty requires little aftercare. I said thereโ€™s a turban wrap that goes around the head and applies pressure on the ears for a few days.

And when that comes off, thereโ€™s ice thatโ€™s put on. Everyone has some bruising and some swelling. Thatโ€™s a given. 

Bruising will fade in two to three weeks. Swelling may take six to eight weeks to resolve.

One of the particularly reassuring things about an otoplasty is that the incision is made on the backside of the ear, and we use dissolving stitches, so no one has to manage them. 

We do put most people on prophylactic antibiotics just to be safe. But apart from icing after the turban is removed, normal activity can be resumed fairly soon. They can wash the areas in one day. So, there is not much maintenance required afterwards.

Are There Any Complications That Patients May Experience During The Recovery From Otoplasty?

The most common complication probably would be infection or what’s called a seroma or hematoma. Infection most people know. Again, we give people antibiotics beforehand, and they take them for a week afterwards.

A seroma is fluid that accumulates under the skin, and a hematoma is blood that accumulates under the skin. If they develop, they will need to be drained. Usually when they are identified, they are manageable to control, and they do not cause any long-term problems.

How Long Before Patients Generally Feel โ€œBack To Normal?โ€

People probably will return to normal within three to six weeks. One of the factors that influences this question is, “When will I look normal?” When can I go out and be in public and have people not know I had surgery?

Well, first of all, we don’t have to worry about the scar. No one will see the scar, so we toss that one out right away.

So, there are two main things to consider: the swelling and the bruising. And when those are gone, then a person can look really good.

So again, probably three to six weeks.

How Many Office Visits Should The Patient Expect?

Typically we see patients in two pre-surgical office visits. The first visit is where we discuss the problem, the concerns and the treatment options.

And then we have another one right before the surgery, which is called a pre-op visit. We like that a week before the actual surgical date. This allows us to iron out any last-minute questions and issues and get everyone on the same page.

Usually the first visit after the surgery is a week postoperatively. If all is well at that visit, then it’s about two to three weeks for the second postoperative visit. If everything looks good at that visit, we will likely schedule one more visit several months later, and then we will discharge the patients.

Are The Effects Of Otoplasty Permanent?

The Davis and Mustarde procedures are both permanent and currently under review. That is 100% true for the Davis procedure because we remove the cartilage and it does not grow back.

If you remember, earlier, the mustard was about the tip of the year being folded over. Re-establishing that fold requires permanent stitches, usually two to three.

if those stitches break for any reason, then that portion of the otoplasty would fail and would have to be redone. Usually, some trauma to the head is needed to impact the stitches and cause them to break.

But in a normal day-to-day situation, you could probably say that the Mustarde procedure is permanent too.
